We understand your time is valuable; our New Patient Form is available online and can be completed directly on your electronic devices. You will know that your form has been successfully submitted to our database when your browser takes you to the THANK YOU page.
If you would prefer, you can also arrive 15 minutes early to your first appointment and fill the forms in-office.

Switching from another dental office?
No problem at all! We appreciate your trust and will coordinate the entire process to make the transfer seamless and simple for you. Contact us and our team member will take care of the rest. We will reach out to your previous dental office on your behalf to request for your dental records and recent x-rays to be transferred, at no cost to you.
What to expect at your first visit?
It is important to us that your visit feels comfortable and welcoming! From the moment you walk into our office and are greeted by our Administration Team, we want you to feel as if you are part of our extended family. Your dental appointment time will be a special one-on-one visit with Dr. Chloe Xu or Dr. Lisa Mallett and our friendly team. They will take all the time necessary to discuss your dental health concerns, needs, and goals, as well as answer any questions you may have in a comprehensive exam.
